Yep a Pediatrician. I sent the email, but I cant promise a response. She just completed her residency, but still does shifts at the hospital. If she just finished an insane 12 hour shift at the hospital, I have a feeling a lot of my emails get auto-deleted.

Marion....are you to only consider bacterial causes for this assignment? Could it be viral? The most common cause of those symptoms in a child that age other than strep....adenovirus?

I'm no expert Marion...but the 3 you've listed seem most likely, if we are excluding strep and viral causes. Is that all the info on the patient you have? Based on what you've said....Bordetella....pertussis....would have presence of cough???...which is not listed as a symptomCornyebacterium....diptheria.....3rd world country? Neisseria....most likely....consider child abuse??

Ok, here's the best my sister could come up with. Dont know if this helps or not. And btw, I didnt tell her what you thought it was. I just told her the symptoms.

Anyway, there's not enough info here to say exactly what it is, but here are a few choices:1.  Corynebacterium diptheria classically causes bilateral tonsil membrane that is whitish-grey that bleeds when swabbed.  Also causes lymphadenopathy and fever and neurological complications 2 -3 weeks after infection (hence the lethargy).  However, everyone is immunized in the US, so it is VERY rare here.2.  EBV or Mononucleosis, although pt is a little young for this- it is typically seen in older kids3.  CMV mononucleosis.  rare also - it is often confused with EBV mono.  4.  Viral - such as Adenovirus or Coxsakie virus, these usually have rash too.5.  Leukemia/ lymphoma ia always considered with prolonged fevers and lymphadenopathy.    That' s all I can come up with.
