The "time out" thing is there precisely to allow the inexperienced players to double check with their captain if they are doing the right thing. The guy at the table looked round to his captain, who was the all round good guy and experienced player Jan Heitmann, and Heitmann saw his player in what he thought was a tough spot. Heitmann agreed to a time out.
It seems likely that the player was simply not at all comfortable putting all his team's chips in the pot without getting his captain's OK. So he got the all clear, returned, and in they went.
It's hilarious, but it's entirely unintentional. The German guy really didn't have any idea that this kind of thing was unethical. As difficult as it might appear to believe, he really didn't know what to do.
As for Pagano, his reaction comes from a conversation he was having with Heitmann before the event about the potential for hilarity from using the time out at inopportune moments. This was pretty much the precise thing they were talking about.
As one last point, Heitmann apologised unreservedly to the Canadian team for any offence caused. And he bought them all a drink. The Canadians took it all in the best spirits and didn't even think an apology was necessary.
